If you believe an item on your statement is in error, complete this form. Please keep a copy for your records. To protect your billing rights, you must report your dispute to us in writing within 60 days of the date of the statement on which an alleged billing error appears. If your dispute is a quality of goods or services issue, you must make a good faith attempt to resolve the issue with the merchant before we can begin an inquiry.
